Chapter 165 Watch the fun

In the morning, Sun Lien walked to the hospital, and it was another day that was no different from usual. He came to the emergency room early in the morning and handed over to his colleagues from last night shift. The next time was a less urgent consultation time. There were shifts on the left and right, and Sun Lien prefers to have patients to see continuously - this would not be too boring.

However, the situation was not satisfactory, as if it was snowing for several consecutive days, and it seemed that it was a working day. The ninth clinic responsible for complex diseases was not open all morning. The emergency room was still busy, but most of the other patients went directly to the outpatient clinic - those who needed to see the emergency room went directly to the emergency room, and it was basically not Sun Lien's turn to deal with it.

After sitting in the ninth clinic for two hours, Sun Lien put down his cell phone with 45 power in his hand. No, he had to find something for himself.

So, this person is born with a bad bone. He is usually busy crying and shouting, but when he is really busy, he has to find something for himself when he has nothing to do. Otherwise, men are all big pig's hooves, right?

Strictly speaking, the doctors who are on standby cannot wander around. However, it seems that the chance of opening the ninth clinic today is not very good. Coupled with the phone number at the door and the doctors in several clinics around, Sun Lien's job of finding a job for himself became natural.

Sun Lien, who was looking for trouble for nothing, was wearing a white coat, with his hands behind his back, and naturally camels a little on his upper body, then shook a pair of black sneakers on his feet and walked towards the rescue hall - his steps were steady and not big, which was the speed suitable for walking after meals.

If you want to watch the fun, you have to go to the emergency hall. If you want to secretly observe and imagine various stories between people, you must go to the infusion room. If you just want to chat with people, go to the hospitalization department and catch two patients who smoke illegally on the rooftop and balcony, they can tell you four or five hours of life experience.

The hospital is actually a big story box, and those patients have the wonderful beginnings of one after another. Most of them have stories far from the end. Sun Lien had this habit during his internship. He liked to look at these patients and guess their stories.

Always remind yourself that you are treating a living person, and Sun Lien always thinks this is very important.

You are simply not human! Sun Lien has reached the infusion area. In response to national requirements, the outpatient clinic of the Fourth Hospital has long cancelled outpatient infusion - patients need to go to a secondary hospital or community hospital for infusion treatment. But the situation in the emergency room is still a little different. Patients who have received hundreds of beds in the emergency room can of course be treated with infusion, while patients diverted from the emergency clinic can also be treated with infusion in the observation area. Of course, this is not the point. The key is that the loud scolding that Sun Lien heard came from a young male nurse.

The male nurse was wearing a light blue male nurse uniform, holding a bag of intravenous infusion liquid covered with a black plastic bag. His eyes were wide open, and his trembling fingers pointed at the nose of an old lady in her sixties. What are you going to do?! Do you know that this will be dead?!

Sun Lien was shocked when he heard this, and his hand behind him also released, Wei Tuo's back straightened, and his steps were walking very fast. What's wrong?

Dr. Sun. The male nurse recognized Sun Lien. When he saw Sun Lien coming, his voice trembled. She spent a long time, but she still didn't say what was wrong.

The old lady was probably a little confused. The middle-aged woman sitting next to her receiving infusion shook her head and told Sun Lien what happened.

The story itself is actually quite simple. The old lady was admitted to the hospital for treatment this morning due to difficulty breathing, coughing, stomachache and poor mental health. After the emergency department conducted a preliminary examination on the old lady, it was determined that she was dyspnea caused by bronchial asthma in the elderly. Therefore, the old lady was prescribed a treatment prescription for aminophylline injection intravenous 2ml (0.25g) and cimetidine 2ml (0.2g) each with 200ml of 5 glucose intravenous infusion.

What the male nurse was holding was a bottle of levofloxacin.

When I checked the patient's name, she actually agreed! The male nurse finally calmed down from fear and anger. I asked her three times in total, but it was not her name three times, but she agreed! If I hadn't had some impression of this old lady having used aminophylline, I would have hung this bottle of levofloxacin on her!

The old lady probably knew that she hadn't understood and agreed and almost caused a big disaster, but she still said stubbornly, I was the third infusion before, and I thought it was my turn to be in your third bottle.

Only then did Sun Lien realize why the male nurse was so nervous?

Aminophylline is an antiasthma drug that has been used clinically for many years, and is also the most widely used classic drug for asthma. However, the effective treatment concentration of aminophylline is very low, and the individual blood drug concentration varies greatly. It is a drug with a narrow treatment window, so blood drug concentration testing is required to ensure that the drug level will not be too high in the patient's body to produce adverse reactions, or it will not be too low to achieve the therapeutic effect.

As for cimetidine, it is a commonly used combination drug in clinical practice with aminophylline. This h2 receptor anti-caking agent can inhibit gastric acid secretion, prevent or reduce gastrointestinal ulcers caused by asthma patients when using aminophylline. Theophylline itself will destroy the gastric mucosal barrier and stimulate gastric acid secretion. Moreover, asthma patients often suffer from gastrointestinal ulcers. On this basis, cimetidine is often regarded as a necessary supplement for the treatment of asthma.

However, when cimetidine and aminophyline are dissolved in 5 glucose solution at the same time, the compatibility stabilization time is only four hours, so it cannot be used safely in clinical practice. Therefore, cimetidine infusion can only be performed after the ammonophylline infusion is completed.

The bottle of levofloxacin covered by a black plastic bag belongs to fluoroquinolones antibiotics. Like the antibiotics of macrocyclic lipids, when used in combination with aminophylline, it will lead to a reduction in the clearance of theophylline, thereby increasing the serum concentration of theophylline.

As mentioned just now, if the male nurse actually mistakenly injected levofloxacin into the old lady, the concentration of theophylline in her serum will not suddenly increase, but will remain at a relatively high level for a long time. After she completes the infusion and walks out of the hospital, she may have serious side effects or be poisoned due to the higher blood concentration of theophylline. The consequences are simply unimaginable.
